APPENDIX A NOTICE OF VIOLATION Georgia Power Company License No. DPR-57 Based on the NRC inspection July 7 - July 10, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by Technical Specification 6.8.1, written procedures are to be established and implemented covering refueling operations and surveillance and test activities of safety-related equipment. Further, Technical Specification 6.8.3 specifies that temporary changes to these procedures shall be approved by two members of the plant management staff, at least one of whom holds a senior reactor operator's license on the unit affected.
Contrary to the above, 1.
On July 4, 1979, during the performance of HNP-1-10110, " Fuel Movement Procedure" a change was made to the fuel load sequence without approval of the required personnel.

During the June 27, 1979 performance of the instrument and system response check portion of HNP-1-3052.
"lRM Instrument Functional Test" the performing operator omitted ce: tain steps applicable when the Reactor Mode Switch was in the REFUE! position. Review of test results by cognizant supervisory personnel on July 2,1979 also failed to identify this discrepancy.
This item is an infraction.
B.
Technical Specification 3.10.A.! requires in part that the Reactor Mode Switch shall be locked in the REFUEL position during core alterations.
Contrary to the above, durin;, core alterations on July 8, 1979, the Reactor Mode Switch was observed to De in the REFUEL position; however, the operate key was found inserted, thus allowing the switch the capability of being manipulated.
