Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-373 Docket No. 50-374 Based on the inspection conducted on June 27 and 28, 1979, it appears that a.e of your, activities was not conducted in full compliance with NRC astirements as noted below. This item is a deficiency.
10 CFR 50, Appendix B, Criterion 1 requires, in part, that measures shall be established to assure that special processes including welding, are controlled.
Contray to the above, it was determined that the various levels of reviews failed to identify that Zack Welding Procedure Specification M-B-QCP22, Revision 1, dated January 21, 1978, prequalified to American Welding Society D1.1-75, inaccurately stated that it was qualified to Paragraph SE (relates to procedures by qualification tests) instead of SA (relates to qualified procedures).
