Corner and Lada Company, Inc.

Cocket No. 99900349/79-01 NOTICE OF DEVfATION Based on the results of an NRC inspection conducted on April 2-6, 1979, it appeared that certain of your activities were not conducted in full compli-ance with NRC requirements as indicated below:

A.

Criterion VII of Appendix B to 10 CFR 50 states in part, " Measures shall be established to assure that purchased material, equipment, and services.... conform to the procurement documents. These measures shall include provisions, as appropriate, for source evaluation and selection, objective evidence of quality furnished by the contractor or subcontractor, inspection at the contractor or subcontractor source, and examination of products upon delivery...."

QA Manual Section QN-3.0, paragraph 3 states in part, "... Purchase orders for code work material shall include the letter 'N' after the purchase order number... Purchasing shall place orders only on vendors which are listed on the Approvei Vendors List (AVL)... Ven-dors holding an ASME Quality System Certificate (Materials),... may be placed on the AVL... All other vendors on the list shall have been surveyed and approved by the Quality Assurance Manager... Ven-dors approved by Guality Assurance shall have been surveyed,... and the results tabulated, updated, and filed on Form CL-65 ' Vendors Survey.'"

Contrary to the Sove, "N" purchase orders were approved by Quality Assurance and pls.d with vendors who were not ASME Certificate holders, nor were they list td on the AVL at the time of the purchase order placement.

In addition, the AVL dated March 1,1979, listed Repablic Steel as having been surveyed on August 9, 1978.

However, form CL-65,

" Vendors Survey," could not be located in the files.

(See Details Section, paragraph C.3.a.)

B.

Criterion V of Appendix B to 10 CFR 50 statcs in part, " Activities affecting quality shall be prescribed by documented instructions, pro-cedures or drawings, and shall be acccmplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for detennining that important activities have been satisfactorily accomplished."

QA Manual Section QN-4.0, paragraph 4 states in part, "... Activi-ties operators shall initial and date the Traveler upon ccmpletion of each operation in the fabrication sequence.

Fabrication may proceed only on items which have successfully passed a required inspection as indicated on the Traveler by the inspector's mark.

7908220 M Contrary to the above:

1.

Traveler operation number 3. ' Tack Wels," for Shop Order 6-7467N, had been completed and signed off by inspection (QC), but had not been initialed 'nd dated by ?he activities operator (welder).

2.

Traveler operation numbers 3 and 4, " Weld Joint 1 and 2,"

for Shop Order 6-7116N had been completed but not initialed and dated by the activities operator (welder). Operation number 5, an inspection hold point, had not been signed off and work continued as indicated by the signing off of operation number 8.

C.

Criterion V of Appendix 3 to 10 CFR 50 states, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be ac-complished in accordance with these instructions, procedures, or drawings.

" Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished.

QA Manual Section QN-5.0, paragraph 3, states in part, "All welding shall be performeo to qualified welding procedures which detail all essential and non-essential process variables and which apply to a specific weld process... The welding procedure to be used for each joint shall be indicated on the Traveler by the welding engineer."

Contrary to the above, flux core arc welding (FCAW) was not being p,_r-fonned in accordance with certain non-essential variables, as detailed in the welding procedure required by the traveler.

FCAW procedure ST-508 requires a travel speed of 7-15 inches per minute, and a contact tube to work distance of 1 - ik inches.

FCAW was observed being per-formed on Shop Orders 6-7491 and 6-7503, using 18 inches per minute travel speed and a 3/4 inch contact tube to work distance, and 18 inches per minute travel speed and a 1/2 inch contact tube to work distance, respectively.
