in the subject inspection.

DEFICIENCY 10CFE50, Appendix 3, Criterion V, states, in part, " Activities affecting quality shall be prescribed by docu=ented instructions... and shall be accomplished in accordance with these instructions,, procedures..."

The licensee's TSAR QA Program Section 17.1.5.1, states, in part, "The Met-Ed Qnali ty Assurance Plan... contains the require =ent that activities affecting quality be docu=ented by instructions, procedures..

This requirement is i=plemented by the major contractor..."..

Burns and Roe, Inc. Specification 2555-69A, Section 3.7.4.5, regarding Instrumntation and Control Equipment Installation states, in part, that ec=pression fittings shall be installed in accordance with the manufacturer's instructions." The =anufacturer's instructions for installing instru=entation tube fittings cpecify a 1-1/4 turn for tightening of co=pression fittings after establishing the starting point using a scribe = ark.

Contrary to the above, as of September 13, 1977, instru=entation tubing pressure connections involving co=pression type connections were not installed in accordance with the manufacturer's instructions as b

evidenced by the lack of the referenced scribe = arks and quality control docu=entation.

Crier, Director GCL 1493 CCPREC*1VE ACTION TAKEN The conpressics type connections on instrumentation tubing which have already been insta' led are subject to hydrostatic testing by the site Startup and Test Orcup. Any connections which had been inadequately tightened would have resulted in leaks during the hydrostatic test and would have been subsequently tightened. Therefore, these cc=-

pressien type fittings which were not instnlled in accordance with the anufacturer's recennendation are satisfactory for their intended service.

CC?2ECTIVE STEPS TO 3E TAR.EN TO AVOID FURTHER VIOLATIONS The contractor who had perforned the work is no longer on site.

The naintenance contractor who now has jurisdiction over this work has been advised of this deficiency and has been instructed to follow the nanuf acturer's instructions f or any future installation. Final acceptance of each joint, however, vill re=ain as a successful hydro-statie test.

Site Procedures are being nodified to incorporate these requirements.

Any such application must be accompanied by an affidavit executed by the owner of the information, which identifies the document or part sought to be withheld, and which contains a statement of reasons which addresses with specificity tile items which will be considered by the Comission as listed in subparagraph (b) (4) of Section 2.790. The information sought to be withheld shall be incocporated as far as possible into a separate part of the affidavit.

If we do not hear from you in this regard within the specified period, the report will be placed in the Public Document Rocm.
