Reactor trip caused by high steam flow with low Tavg resulting from unequal pressure between steam generator and header and rapid header isolation valve opening.

Malfunction of EHC caused turbine to roll at a high rate as the governor valves opened spiking 1st stage pressure above 15 per cent which tripped turbine.

Turbine trip, caused reactor trip.
