RIS 2005-29 was initially issued to discuss concerns with demonstrating that certain events would not escalate into more serious events.

L2017-CRS- IN Boiling-Water Reactor Dryout Indications Occurring During Normal Operation 20 07/28/2017 0046 This IN is intended to inform addressees of recent operating experience related to numerous dryout indications which occurred over multiple cycles at the Leibstadt Nuclear Power Plant (NPP), a BWR/6 in Switzerland.

L2017-CRS- RIS Electronic Signature Use for Medical Licensee Internal Documents Required by NRC Regulation 19 08/24/2017 0048 This RIS is intended to supply information to assist medical use licensees in complying with the current NRC requirements related to the use of electronic signatures in internal licensee electronic documents and to clarify what constitutes a signature on electronic documents that medical use licensees are required to sign and maintain.

L2018-GEN- RIS Preparation and Scheduling of Operator Licensing Examinations 3 12/19/2018 0010 This RIS is intended to inform addressees of the NRC staffs need for updated information on project site-specific operator licensing examination schedules and on the estimated number of applicants planning to take operator licensing examinations. This information will help the NRC plan its resources more effectively.

L2018-GEN- IN Operating Experience Regarding Flow-Accelerated Corrosion 3 12/19/2018 0012 Flow-accelerated corrosion (FAC) is an industry issue that was first recognized to be of significant concern at Surry in 1986. There continue to be FAC failures attributed to problems with the FAC monitoring program and its software that have caused reactor trips.

(Mon) OPENED L2019-GEN- IN: Effective Cyber Security Practices for Consideration to Protect Digital Assets of Byproduct Materials 2 01/15/2019 0000 Licensees The purpose of this IN is to inform affected byproduct materials licensees of the results of an evaluation that was previously conducted by NRC staff of the potential need for cyber security requirements and of effective cyber security practices for licensees consideration. The INs enclosure will include the effective practices that provide the addressees information on contemporary cyber security issues related to the protection of digital assets, including those assets used to facilitate compliance with physical security requirements such as Title 10 of the Code of Federal Regulations (10 CFR) Part 37, Physical Protection of Category 1 and Category 2 Quantities of Radioactive Material.

L-2019- IN: Potential Over-Pressurization of High Specific-Activity Alpha-Emitting Radioactivity Sources 2 01/15/2019 GEN-0006 A liquid source containing 1.5 mCi (55.5 MBq) of Americium 241 was stored in a sealed glass storage container for 12 years, which burst in August 2017, causing significant personnel exposure risk. The cause of this container failure is believed to be the generation of hydrogen gas through hydrolysis, which caused pressure buildup in the container, and which is likely to be a problem for similar high specific-activity alpha-emitting radionuclides in an aqueous solution.

L-2019- IN: Issues With Receipt of Security-Related Generic Communications By NRC Licensees That Possess Risk- 2 01/15/2019 GEN-0003 Significant Quantities Of Radioactive Material Staff response to feedback from stakeholders during the Part 37 program review that some licensee personnel responsible for managing Part 37 requirements at their facilities do not receive security-related NRC generic communications L-2019- IN: Patient Skin Contamination Events Associated With I-131 Mibg During Neuroblastoma Treatments 2 01/15/2019 GEN-0004 The information notice alerts licensees to the potential patient contamination risks associated with therapeutic treatments with iodine-131 meta-iodobenzylguanidine (I-131 MIBG). The NRC expects that licensees will review the information for applicability to their facilities and consider actions, as appropriate, to avoid similar problems.

L-2019- IN: Recent Medical Events Associated With Yttrium-90 Microsphere Brachytherapy 2 01/15/2019 GEN-0005 The information notice alerts licensees to the potential for medical errors when the written directive is not properly completed or verified. The NRC expects that licensees will review the information for applicability to their facilities and consider actions, as appropriate, to avoid similar problems.
