_ This refers to the Nuclear Regulatory Commission (N.RC) inspection conducted by*

Mr. M. W. Branch of this office on November 1 through 30, 1991.

The inspection,

included a rev*iew of activities author*ized for your Surry facility.

At the

conclusion of the inspection, the findings were discussed with those members of

your staff identified in the enclosed inspection report. _

Within

  • these areas, the inspection consisted of selective examinations of procedures

and representative records,* interviews with personnel, and observation of

activities in progress..

Baied on the results of this inspection, certain activities appeare~ to be i~

violation of NRC requirements, as specified in the enclosed Notice of Violation

(Notice).

We are concerned about the apparent programmatic na_ture of the _

containment isolation_ valve violation.

Since your plant was designed and

contructed prior to the issuance of the General Design Criteria, it i.s

important that the approved containment isolation configuration is acc.urately

captured in a licensing basis document.

You are required to respond to this letter and should follow the instr~ctions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

In addition, based on the potential

programmatic nature of this violation, we request that you address the

programmatic aspects of this* issue in your response.

After reviewing*your

tesponse to this Notice, including your proposed corrective actions and the

results of future inspections, the NRC -will .determine whether further NRC

enforcement action is necessary to ensure compliance with NRC regulatory

requirements.

In accordance with 10 CFR 2.790 of the NRC's

11 Rules -of Practice
