This refers to the Nuclear Regulatory Cammi ssion (NRC) inspection conducted
by W.
E. Holland, L. E. Nicholson, and J. W. York, on October 29 through
November 25, 1989.
The inspection included a review of activities
authorized for your Surry f aci 1 i ty.
At the cone 1 us ion of the inspect ion,
the findings were discussed with those members of your staff identified in
th~ enclosed inspection report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to
violate
NRC
requirements.
The violations,
references to pertinent
requirements, and elements to be included in your response are described in
the enclosed Notice of Violation.
As noted in this report, you have sustained several instances of failure to
samp 1 e the effluent service water from your component coo 1 i ng water heat
exchangers.
We note that the need to sample this system is due to
long-standing inoperability of the process monitoring instrumentation.
We are
concerned with the material condition of this equipment.
As such, in addition
to your response to Violation C in the enclosed Notice of Violation, we request
that you address your program to assure ~hat all process monitoring instrumen-
tation is maintained operable and also the specific actions you are taking to
return the Surry inoperable process instrumentation to service in an expedi-
tious manner.
In accordance with
Section 2.790 of the NRC's "Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
The responses directed by this letter and its enclosures are not subject to
the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
