This letter refers to the Enforcement Conference held on October 17, 1989, at the Region II Office.

The meeting concerned activities authorized for your Surry facility and involved several maintenance/corrective action issues and also radiation area access control problems.

A list of attendees, a meeting summary, and a copy of your handout are enclosed.

ENCLOSURE 2 ENFORCEMENT CONFERENCE

SUMMARY

An NRC and Virginia Electric and Power Company (Virginia Power) Enforcement Conference was held at the Region II office on October 17, 1989.

The subjects discussed involved various maintenance issues which exhibited a failure to preclude repetition of conditions adverse to quality and also examples of failure to comply with radiation protection access control requirements.

The Regional Administrator began the Enforcement Conference by expressing concerns with the licensee's ability to assure that first-line supervisors are actively pursuing and promoting maintenance quality assurance and radiation access control practices to plant personnel.

He further stated that it was a positive indication that the licensee's Quality Control Department found the majority of the maintenance problems and that Health Physics (HP) personnel were performing at a high level. The Vice President -

Nuclear Operations for Virginia Power agreed with the remarks and stated their commitment to correct the corrective action implementation problems for these issues.

The Surry Station Manager continued the meeting by discussing the three maintenance/corrective action

issues, i.e.,

flow orifice installation, foreign material exclusion, and torquing control, using the enclosed handout (Enclosure 3).

These issues were previously discussed in detail at a Manage-ment Meeting held at the Region II office on September 1, 1989, and sum~arized in a Meeting Summary to Virginia Power dated October 2, 1989.

The causes of the maintenance problems were inconsistent implementation of administrative requirements, personnel error, ineffective maintenance standing orders, and weaknesses in line management reviews.

The Station Manager agreed that Virginia Power has not fully implemented their commitment to complete thorough corrective action related to the maintenance problems and expressed his intentions of making all levels of plant personnel aware of corrective action

'implementation.

The HP concerns, which involved various failures to comply with Virginia Power radiation protection rules and NRC regulatory requirements, were then discusssed by Virginia Power and NRC Management.

The discussion focused on two of the.more recent occurrences; the first involving a contractor who willfully circumvented a locked high radiation barrier (locked plate on ladder) to gain entry to the excess letdown flats on the 13-foot elevation of the Unit 2 containment, and the second involving two contractors who entered a high radiation area without a dose rate meter or HP coverage as required.

The individual doses were determined to be 3 mrem for the first event, and for the second event, 3 mrem and 5 mrem.

In both occurrences, HP personnel discovered the problem and took immediate corrective actions.

Upon review of these events, Virginia Power management concluded that although high radiation areas are posted properly, access control procedures are clear, and appropriate worker training is accomplished concerning access requirements, certain individuals are not concerned with paying attention to detail.

The Surry Station Manager further stated that when workers are found to have violated radiation access control, they are dealt with severely.

In addition to improved

Some long-term corrective actions mentioned included the incorporation of audio radiation area protection signs and a possibility of acquiring alarming dosimeters for the site.

Virginia Power management concluded by saying that effectiveness of program management will be placed on line managers and performance-based reviews will be conducted to ensure that proper knowledge of controls and practices are provided to the plant staff.

In addition, INPO based self-assessment criteria will be developed in order to give good feedback to management.

A long-term goal is to develop a trending program for various programs to improve review effectiveness.

The Regional Administrator closed the meeting by thanking Virginia Power for attending and reiterated the positive aspect of licensee 1 s identification of most of the problems and encouraged specific attention to the performance of first-line management supervision.
