ENCLOSURE 2' MANAGEMENT MEETING

SUMMARY

.. A~ NRC and Virginia Electri~ and Power Co~pany (VEPCO) Management Meeting was.

held at the Region II (RII} office on September 1, 1989,. to discuss the status.

. of the March 9, 1989, Confinnation of Action Letter {CAL) items for* Surry Unit 2 restart.

An, overview discussion of various ma.intenance-issues which have been* identified was also part of the agenda.

Fol lowing brief opening remarks by NRC and VEPCO,. the Surry Sta ti on Manager began the presentation, using the enclosed handout as. {Enclosure 3), by giving a synopsis of three maintenance problems which have been repetitive in nature.

The first of these issues involved the installation of reversed flow orifices.

After identification of the* problem*, further inspections revealed six

additional reversals *.

  • The Surry Station Manager stated that none of the
  • reversed orifices found had operational concerns.

Corrective actions for these occurrence~ would include revisions to procedures in order to. add verification steps of procedure adherence.

A brief discussion was held on reversed orifices as they related to the North Anna facility.

The inspections to date at North Anna. found.* results similar to Surry's~ findings with no operational coi:icerns noted.

The majority of the reversed orifices found* at North Anna affected local flow-indications only.

The, second maintenance issue involved foreign material exclusion~

A recent event involved the dropping of a foreign object (bolt) into a safety* injection* check valve.

The Station Manager detailed the root cause~ of the event which involved both failure to fol lo~ procedures and inadequate procedures and also detailed*the corrective actions taken to. date.

The third maintenance issue involved the requirements for torquing control.

A recent event occurred where the licensee had to recheck the material properties inside* certain safety injection check valves to verify proper torquing require-ments.

A review was. presented of the provision of VEPCO I s current torquing control program, which concluded that the overall program was adequate; however, implementation was inconsistent and not complete.

The Station Manager stated that the,* standing order for maintenance torquing requirements wi 11 be in fu 11 compliance by-October 31, 1989.

The Director of the Division of Reactor Projects (DRP), RII, stated that the* previously discussed maintenance issues will be* further reviewed by NRC management for possible* escalated enforcement actions regarding corrective action timeliness and: adequacy, and. also that these-issues. w-ill,. in part,. be* the subject of a forthcoming. NRC Maintenance*
