ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power Company Surry Uni.t 1 Docket No. 50-280 License No. DPR-32 During the Nuclear Regulatory Commission (NRC) inspection conducted between the period of July 2-29, 1989, violations of NRC requirements were identified.
In accordance with the 11General Statement of Policy and Procedure for NRC Enforcement Actions, 11 10 CFR Part 2, Appendix C (1989), the violations are 1 i sted be low:
A.
Technical Specification 6.4 requires detailed written procedures with appropriate check-off lists and instructions shall be provided and followed for calibration of components involving the nuclear safety of the station.
Contrary to the above, on July 9, 1989, detailed written procedures and/of instructions were not provided in that incorrect calibration equipment was us~d to adjust the flux trip setpoint on the Unit 1 power range nuclear instrumentation channe 1 N41.
Use of the incorrect instrument resulted in a b 1 own fuse and subsequent transient causing an automatic reactor trip from approximately 30 percent power.
This violation is a Severity Level IV violation (Supplement I).

Technical Specification Table 3. 7-2, Item 5.a requires that an inoperable low intake canal level channel be placed in trip within one hour after the channel becomes inoperable.*
Contrary to the above, on July 14, 1989, stop logs were installed between the channeJ I II 1 ow intake canal 1 eve 1 instrument and the intake canal rendering the channel inoperable.
The channel was not placed in trip as required until approximately seven hours after the instrument became inoperable.
This violation is a Severity Level IV violation (Supplement I).
C.
Technical Specification 3.6.B.2 requires that a minimum of 60,000 gallons of water shall be available in the tornado protected condensate storage tank of the opposite unit to supply emergency water to the auxiliary feedwater pump suction of that unit.
Contrary to the above, on July 27, 1989, the Unit 2 emergency condensate storage tank (2-CN-TK-1) contained only approximately 8909140003 890901 PDR ADOCK 05000280 Q

-Docket No. 50-280 License No. DPR~32 50,000 gallons of water for approximately 4 and 1/2 hours due to operator error.
During this time, Unit 1 was operating at power requiring the quantity of water specifie~ above to be available.
This violation is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC
- 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a 11 Reply to a Notice of Violation 11 and _should include for each violation:
(1) admission or denial of the violatfon, (2) the reason for the violation if admitted, (3) the corrective steps which have be~n taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or _why such other action as may be proper should not be taken.
