NRC INSPECTION REPORT NOS .. 50-280/88-45 AND 50-281/88-45

This refers:;to the Nuclear Regulatory Commission (NRC) inspection conducted by

W. E. Holland and L. E. Nicholson on November 6 through December 17., 1988.

The inspection included a review of acti vi ti es authorized for your Surry

facility.

At the conclusion of the inspection, the findfngs were discussed

with those members of your staff identified in the ~nclosed inspection report.

Areas examined during the inspection are identified in the_ report.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements.

The violation identified in para~raph 4 of the enclosed

report is being-considered for escalated enforcementcaction.

Accordingly, no

Notice of Violation is being issued at this time for this violation and no

response is required.

However, please be advised that the characterization of

this violation described in the enclosed Inspection Report may change as a

result of further NRC review.

You will be advised by separate cortespondence

of the results of our deliberations of this matter.

We are satisfi~d with the effectiveness of your corrective action program as

discussed in the meeting in Atlanta on January. 5, 1989.

Of course, we will be

inspecting the implementation of that program to assess its effectiveness with

respect to providing reasonable assurance that train and system independence

and configuration have been achieved in accordance with design for all

critical safety systems.

Please provide us your assessment of the effective-

ness of this program within 30 days of its comp 1 eti on.

During the course of the inspection, the inspectors identified that a problem

with ra*in water leaking into the safeguards room and wetting the auxiliary

feedwater pump motors has continued.

This water intrusion has occurred a

number of times during recent years and has resulted in your declaring these

pumps inoperable.

As discussed in the eiit interview, we are concerned with

your lack of aggressiveness to implement a long lasting solution to this

problem.

As part of the review of LER 281-88-01, we noticed that a comprehensive root

cause evaluation could not be performed because the replaced parts were

discarded.

In your 30-day response requested above, please provide actions that you are

planning to implement to resolve the concerns noted above.

In accordance with

Section 2.790 of the NRC's "Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its

enclosure will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosures are *not subject to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
