ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric & Power Company Surry 1 Docket No.: 50-280 License No.: DRP 32 During an NRC Inspection conducted on September 5 through October 2, 1993, a violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

10 CFR 50, Appendix B, Criterion V, as implemented by Operational Quality Assurance Program Topical Report (VEP l-5A, Section 17.2.5) requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance

  • with these instructions, procedures, or drawings.

Operations Program Administrative Procedure (OPAP)-0010, Tag-Outs, dated March I, 1993 requires tagging records to be established and implemented and the qualified operator, assigned the task of independent verification, shall verify alignment of the components listed on the tagging record. Tagging record number (S)l-93-EP-0053 was established on September 28, 1993, to prescribe removal of fuses, l-EP-FUSE-EH-14Hl-6 and l-EP-FUSE-EJ-14Hl-6, on safety-related load center IHI.

Contrary to the above, on September 28, 1993, activities affecting quality were not accomplished in accordance with prescribed instructions, in that, fuses l-EP-FUSE-EH-14H-6 and l-EP-FUSE-EJ-14H-6 were removed in place of the specified fuses and the components' alignment was not properly independently verified.

As a result, the A train Low Head Safety Injection Pump supply breaker trip circuit and the A train Inside Recirculation Spray Pump supply breaker closing circuit were rendered inoperable for several minutes.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Virginia Electric and Power Company is hereby required to submit a written statement or explanation to the US Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the 9311160199 931028 PDR ADOCK 05000280 G

If an adequate reply is not received within the time specified in this Notice, an order or Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as my be proper should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
