ENCLOSURE 1 NOTICE OF VIOtATION Virginia Electric and Power Company Surry Units 1 and 2 Docket Nos. 50-280, 50~281 License Nos. DPR-32, DPR-37 During the Nuclear Regulatory Commission (NRC) inspection conducted between the period of June 5 to July 2, 1988, a violation of NRC requirements was identified.

The violation identified involved the failure to provide adequate detailed instructions that resulted in several flow orifices being installed backwards.

In accordance with the 11General Statement of Pol icy and Procedure for NRC Enforcement Actions, 11 10 CFR Part 2, Appendix C (1987), the violation is listed below:

Technical Specification 6.4 requires in part that detailed written procedures, appropriate check-off lists, and instructions shall be provided for preventive or corrective maintenance operations which could have an effect on the safety of the reactor.

Contrary to the above, prior to June 10, 1988, appropriate instructions were not provided for the assembly of flanges containing non-reversible orifices used for flow indication, resulting in at least eight flow orifices being installed backwards.*

Also, instructions to craft personnel.were inadequate with regards to fastener thread standout~

This is a Severity Level V violation (Supplement I), and applies to both units.

The above violation was identified to Vir.ginia Electric and Power Company during the inspection period; and, prior to the end of the inspection period, they had taken extensive corrective actions to prevent recurrence.

After discussion of the violation and corrective actions between the inspectors and NRC management, it was concluded that the violation was considered a Severity Level V categorization, and that no written statement or formal response would be required.
