Gentlemen:

Subject: Repo~t Nos. ~and 50-281/81-25

This refer*s to the routine safety inspection conducted by Mr. J. M. Puckett of

this office on September 21-25, 1981, of activities authorized by NRC Operating

License Nos.* DPR-32 and DPR-37 for the Surry facility. Our preliminary findings

were discussed with Mr. J. L. Wilson, Station Manager, at the conclusion of the

inspection.

The inspection was an examination of the activities conducted under your license

as they relate to*radiation safety and to compliance with the Commission's rules

and.regulations and the conditions of your license. The inspection consisted of

selective examinations of procedures and representative records, interviews with

personnel, and observations by t~e inspector.

During this inspection it was found that certain activities under your license

appear to violate NRC requirements.

These items and references to pertinent

requirements are listed in .the Notice of Violation enclosed herewith as

Appendix A.

Elements to be included in your response are delineated in

  • Appendix A.

We have.examined actions you have taken with regard to previously identified

enforcement matters. These are discussed in the enclosed inspettion report.

In accordance with 10 CFR 2.790(a), 'a ~opy of this letter and the enclosure(s)

will be placed in the NRC's Public Document Room unless you notify this office,-_

by.telephone, within ten days of the date of this letter and submit written

app.lication to withhold information contained therein within thirty qays of the

date of thii letter. Such application must be consistent with the re~uirements

of 2.790(b)(l).
