This refers to your letters of December 31, 1981, and January 15, 1982, informini
us of steps you have taken to correct the violations concerning activities under
NRC Operating License Nos. DPR-32 and DPR-37 brought to your attention in our
letter of December 1, 1981.
We will examine your corrective actions and plans
during subsequent inspections.
In your response, you denied Violation C. and stated that compensatory measures
to maintain administrative controls over entry and egress from the unit one seal
water filter room were in effect in that:
(1) keys are normally attached to the
individual
1 s pers6n in some fashion and the potential for loss of a key is thus
minimal; (2) the seal water filter room is small and individuals entering this
room would not padlock the gate after entering and continuous visual surveillance
of the gate is possible at all times when in the room; and (3) the
11 buddy system
11
is employed and adds a measure of safety to all entries into high radiation are~s
by providing an individual to assist or call for help in the event of a problem
during such entries.
We agree that in this unique situation appropriate controls were established
to permit egress from a high radiation area.
Therefore, Violation C. cited in
Appendix A and transmitted by our letter of December 1, 1981, is hereby withdrawn
and our records will be changed accordingly. It should be noted, however, that
generally we do not consider the practice of using chains and padlocks for
securing high radiation areas acceptable because of the potential of locking
personnel inside.
