CAUSE

There are several manual valves inside -containment which must be *open*in order to perform the month.1-y flushing of sensitized stainless steel pipe.

In order that the number of containment entries could be kept to _:a minimum, thf!J~e manual valves had been *left-open.

With manual valves 2-SI-249 and 2-SI_-256 left open, charging system pressure existed on the down~tr_eam side of check valve l-SI-144 which leaked througj:i, thereby diluting "C" accumulator.

IMMEDIATE CORRECTIVE ACTION

RWST to bring the boron concentration within specification.
