Description of Event

e HEAT TRACING FAILURE With Unit ://:2 at 100 percent power PT-27B revealed that the amp readings for circuit 6A (boric acid to blender) panel ://:11 were below the Acceptance Criteria stipulated in the PT.

Repair of the defective circuit was initiated immediately.

This event is a degraded mode of operation as specified in T.S.3.2.D.3 and is_ reportable in accordance with T.S.6.6.2.b(2).

2.

Probable Consequences & Status of Redundant Systems:

The Heat Tracing is intended to maintain a temperature above that needed for flow.

The redundant Heat Tracing Circuit was operable, the-refore, the health and safety of the public were not affected.

3.

Cause of Occurrence:

The Heat Tracing Tape failed due to excessive heat.

4.

Immediate Corrective Action

The immediate corrective action was to verify that the redundant circuit was operable.

5.

Subsequent Corrective Action:

The defective Heat Tracing Circuit was replaced within the time span specified by the Technical Specifications.

6.

Action Taken to Prevent Recurrence:

No additional actions were deemed necessary.

7.

Generic Implications:

A task force has reviewed the total spectrum of the Heat Tracing system and a Design Change is being prepared as a result of the Task Force Study.

Long lead items are being procured.
