I The operator, when reviewing the Technical Specification Limitations at [III] 1 Shift turnover, failed to note the requirement for increased testing [II}] I frequency for the operable pump.

The immediate corrective action was to [2JJJ I demonstrate operability using Periodic Test 18.1.

FAILURE TO CONFIRM OPERABILITY OF SAFETY INJECTION PUMP

1.

Description of Event

Prior to tagging out the 'B' Low.Head Safety Injection Pump for mechanical maintenance, the redundant rump was demonstrated operable as required by the technical specification 3.3.B.3.

The on coming shift reviewed the minimum equipment list and the technical specification and failed to note the requirement for demonstrating operability of the redundant pump at eight hour intervals.

The error was discovered and the Periodic Test initiated approximately 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and 40 minutes after the 'B' pump was tagged out.

This contrary to Technical Specification 3.3.B.3 and reparable per Technical Spe-cification 6.6.2.b.(3).

2.

Probable Consequences and Status of Redundant Equipment:

The redundant pump, 2-SI-P-lB, was out of service for maintenance.

Although the pump was not demonstrated operable at the specified time, the pump was capable of performing it's designed function as shown by the sub-sequent testing.

The health and safety of the publid were not affected.

3.

Cause

The operator, when reviewing the Tech. Spec. limitations, did not note the requirement for demonstrating operability of this pump.

4.

Immediate Corrective Action

Initiated PT 18.1 to prove operability of 2-SI-P-lA

5.

Subsequent Corrective Action:

None required

6.

Action Taken to Prevent Recurrence:

All license personnel were reinstructed on the necessity of a more complete review of the Technical Specification limitations when a safety system is operating in a degraded mode.
