so 1 The Heat Tracing Tapes were damaged during mechanical work in nearby area.

The 1 damage Heat Tracing Tapes were replaced; Valve MOV-2867D was tested satis-factory and returned to service.

1.

Description of Event

e MOV-2867D INOPERABLE This heat tracing is necessary to allow MOV-2867D and its associated piping to be considered operable.

A redundent flow path for the injection Tank Discharge was immediately verified operable.

With Unit 2 operating steady state at 100%

power, Periodic Test PT 27D was performed.

Low amperage readings were found on circuit lB, panels 10 and 11.

( a redundant flow path in the discharge piping

_from the Boron Injection Tank, BIT).

Since heat tracing is required to prevent Boron from solidifying the loss or reduction of capability of heat tracing results in reduced reliability, thus valve MOV-2867D was declared inoperable.

This contrary to T.S.3.3.A.7 and is reportable as per T.S.6.6.2.b.(2).

2.

Probable Consequences and Status of Redundant Systems:

Heat Tracing is required to prevent Boron from solidifying.

The loss or reduction of capability of the heat tracing results in reduced reliability, thus valve MOV-2867D and piping '.:i"-SI-90-1503 were declared inoperable.

Since valve MOV-2867G and all the common piping of the BIT Discharge were not affected, the health and safety of the public were not affected.

3.

Cause of Occurance:

Valve MOV-2867D and piping 3"-SI-90-1503 were declared inoperable due to mechanical damage to the heat tracing tapes as a result of rriedian_ical damag;°e while erecting scaffolding to accomodate installation of pipe supports. -

4.

Immediate Corrective Action

Pursuant to T.S.3.3.B.4, PT-18.6C was immediately initiated, demonstrating MOV-2867C to be operable.

5.

Subsequent Corrective Action:

Replaced damaged heat tracing tapes affecting MOV-2867D and line 3"-SI-90-1503.

Tested the operation of valve MOV-2867D within the time span specified by tech-nical specification T.S.3.3.B.4.

6.

Actions Taken to Prevent Recurrence:

Construction has been instructed to exercise more care with scaffolding construc-tion.
