APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company Surry 1

. Based on the NRC inspection June 9-30,. 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Technical Specification 6.4.A.7 and 6.4.D, the.preventative or corrective maintenance procedures which would have an effect on the safety of the reactor must be followed.
Electrical Maintenance Procedure EMP-C-EPH-25 and -26 require areas to be properly cleaned, that tools, materials and rags are collected, and that all tray covers are replaced.
NUS 9045.requires permanent tray cover~ to be installed as soon as possible after cable pulls and that dirt, scrap materials, rubbish and flammable refuse not be permitted to accumulate in the trays.
Contrary to the above, during Unit 1 operations on June 16, 1980, various Unit 1 safety-related cable tray covers were found not in place or properly bolted, and rags, papers and prints, tape, and wood were found on several Unit 1 safety-related cable tray covers in the emergency switchgear room and cable vault.
This is an infraction.
