APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company Surry Units 1 and 2 License Nos. 50-280 50-281 JUL 1 9 1979 Based on the NRC inspection June 26-29, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated Decembe.r 31, 1974.
A.
As required by 10 CFR 71.12.b.1.i the person using a package pursuant to the general license provided-by 71.12.b must have a copy of the specific license, certificate of compliance, or other approval authorizing use of the.package and all documents referred to in the license, certificate, or other approval, as applicable.
Contrary to the above, on April 26, 1979, the licensee used a cask licensed under 10 CFR 71.12.b (Certificate of Compliance No. 9122) and did not have the certificate of compliance and referenced documents.
This is a deficiency.
B.
As required by 10 CFR 71.12.b.1.ii, the person using a package pursuant to the general license provided by 71. 12. b must comply with the terms and*
conditions of the license, certificate, or other approval, as applicable, and the applicable requirements of this part.
Certificate of Compliance No. 9122, Condition 5.b,2 allows for only 8000 pounds to be shipped in the package.
Contrary to the above, on April 26, 1979, 9800 pounds were shipped in a package licensed under Certificate of Compliance No. 9122.
