e APPENDIX A NOTICE OF VIOLATION e
Virginia Electric and Power Company Surry Power Station, Units 1 and 2 License Nos. DPR-32 DPR-37 Based on the NRC inspection May 29-31, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by Technical Specification 6.4.D, procedures prescribed by Specification 6.4.B.1, Health Physics Procedures, must be followed.
Health Physics Manual Section 1.3.F.5 requires instructions on a Radiation Work Permit be followed.
Standing Radiation Work Permit JUN 2 0 1979
.No. 79-SWP-7, Station Restricted Control Area (RCA), requires individuals notify H.P. prior to working in any Restricted Control Area and specifies protective clothing and equipment precautions to be "as required by H.P.".
Contrary to the above, on February 20, 1979, a hand hole was cut in a hatch cover in the Fuel Building without prior H.P. notification and determination of protective equipment requirements.
This is an infraction.
