Serial No. 467 PO/FHT:scj Docket Nos. 50-281 License Nos. DPR-37 We have reviewed your letter of June 4, 1979, in reference to the inspection conducted at Surry Power Station Unit No. 2 on May 7-10, 1979, and reported in IE Inspection Report No. 50-281/79-33.
Our responses to the specific infrac-tions are attached.
We have determined that no proprietary information is contained in the report
- Accordingly, the Virginia Electric and Power Company has no objection to this inspection report being made a matter of public disclosure.

- I RESPONSE TO VIOLATIONS LISTED IN IE INSPECTION REPORT NO. 50-281/79-33 A.
NRC COMMENT As required by Criterion V of Appendix B to 10 CFR 50 and as imple-mented by VEPCO Topical Report VEP-1-A, Amendment 3, Paragraph 17.2.5, activities affecting quality shall be prescribed by docu-mented procedures, and shall be accomplished in accordance with these procedures.
Daniel Construction Company Construction Proced-ure (DCC-CP) WP-502, Revision 1, "Control of Welding Filler Material" requires that field electrode control station attendants attach a sketch to the front of each electrode holding oven specifying the size, class, lot and location of the covered electrodes in that oven.
Furthermore, DCC-CP WP-502 Revision 1 requires field electrode control stations to be secured to prevent unauthorized access.
Contrary ~o the above, the following examples were observed:
- 1.
On May 8, 1979 the sketch on the electrodes holding oven in the refurbishment building contained an incorrect lot number~
This incorrect number was also transferred to eight "Field Requisition Welding Materials" (W-100 forms) *
- 2.
On May 10, 1979, the key to the above locked oven was in the possession of an individual not authorized to possess the key.
- 3.
On May 10, 1979 the electrode location and identification sketch for the above oven reflected no electrode on the center shelf when in fact the center shelf contained electrodes.
'nlis is an infraction.
RESPONSE
The above information is correct as stated.
- 1.
Corrective steps taken and results achieved:
(a)
Affected requisitions have been corrected.
(b)
'nle key to the electrode holding oven in the refurbishment building has been removed from a key ring containing other unrelated keys and isolated.
The key is properly in the custody of the Daniel Construction Company Piping Foreman.
(c)
The electrode location and identifications sketch on the electrode holding oven in the refurbishment building has been corrected to reflect the locations of all electrodes in the oven.

Corrective steps taken to avoid further non-compliance:
(a)
The internal method of handling Field Requisitions for Welding Materials (W-100 Forms) has b-een r~fined to insure proper and complete documentation.
(b)
Increased surveillance by Daniel Construction Company and VEPCO Quality Control personnel has been implemented to insure proper control of keys to electrode holding ovens.
(c)
Contractor and VEPCO Quality Control personnel have implemented increased monitoring to insure proper control of electrodes in holding ovens.
- 3.
The date when full compliance will be achieved:
Full compliance has been achieved.

NRC COMMENT As required by Criterion XVII of 10 CFR SO, and as implemented by VEPCO Topical Report VEP-1-A, Amendment 3 Paragraph 17.2.17, sufficient records shall be maintained to furnish evidence of activities affecting quality and that these records include results of inspection.
Contrary to the above, on May 8; 1979 five "Receipt Inspection Report" forms had the following boxes not marked:
"No Shipping Damage", "Proper-ly Packaged", "Cleanliness Satisfactory", and "Properly Identified".
These reports were found in the following purchase order filed: SGR-12188, SGR-12391 and SGR-12609.
This is a deficiency.
RESPONSE
The above information is correct as stated.
- 1. Corrective steps taken and results achieved:
Purchase Order files SGR-12188, 12391 and 12609 have been corrected to reflect complete documentation and to insure completion of the Receipt Inspection Report forms.
- 2.
Corrective steps taken to avoid further non-compliance:
VEPCO Quality Control Personnel have been instructed to insure completion of all appropriate blocks on the Receipt Inspection Report forms prior to acceptance of the forms.

Full compliance has been achieved.
