NRC INTEGRATED INSPECTION REPORT 50-272/98-12, 50-311/98-12

NOTICE OF VIOLATION

Dear Mr. Keiser:

On January 16, 1999, the NRC completed an inspection at your Salem 1 & 2 reactor facilities.

The enclosed report presents the results of that inspection.

During the six-week period covered by this inspection report, your activities at Salem were

generally characterized by safety-conscious operations, sound engineering and maintenance

practices, and careful radiological work controls. Maintenance associated with the 21 reactor

coolant pump seal replacement and the 13 auxiliary feedwater pump failed surveillance test

were well controlled and supervised. However, we also noted a large number of emergent

corrective maintenance issues which resulted in several unplanned technical specification action

statement entries and work week schedule perturbations. Engineering support of operations

was generally good, but there were multiple delays in the test of the control room emergency

ventilation system, which also impacted previously scheduled work.

We are particularly concerned about the circumstances which led to the December 8, 1998,

reactor coolant system (RCS) leak at Unit 2. We concluded that this event was largely the result

of inadequate control room supervision during start up activities from the forced outage.

Additionally, we identified two examples of procedure violations as a result of our review.

Specifically, an inadequately prepared procedure permitted operators to raise RCS pressure to

the pressure relief valve setpoint of the residual heat removal system. Secondly the on-shift

operations superintendent failed to assess the event in a timely manner in accordance with your

emergency plan requirements.

Based on the results of this inspection, the NRG has determined that a violation of NRG

requirements regarding procedures and procedure compliance occurred. The violation is cited /I

in the enclosed Notice of Violation (Notice) and the circumstances surrounding it are described

in detail in the subject inspection report.
