APPENDIX A NOTICE OF VIOLATION*

Public Service Electric and Gas Company Salem Nuclear Generating Station Unit 2 Docket No:

DPR-75 During an NRC inspection conducted from September 13, 1998 through October 24, 1998, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, Revision 1, the violation is listed below:

Technical Specification 6.8.1.a requires in part that written procedures sha*ll be implemented covering the activities specified in Appendix A of Regulatory Guide (RG) 1.33, "Quality Assurance Program Requirements," Revision 2, dated February 1978.

RG 1.33 Appendix A section 1.c requires procedures for safety-related equipment control. Section 8.a requires procedures to ensure that instruments and controls are properly controlled and calibrated.

PSE&G administrative procedure NC.NA-AP.ZZ-0005(0) Revision 9, "Station Operating Practices," section 5.6 requires that auxiliary feed water (AFW) system components being aligned to a final position to support future operation shall receive an independent verific_ation of position status.

PSE&G maintenance departr:nent procedure SC.IC-GP.ZZ-0003(0) Revision 7, "General Instrument Calibration for Field Devices," section 5.1 requires that temporary modifications to affected* systems and devices as a result of implementing the procedure shall receive independent verification.

Contrary to the above, on July 25, 1998, it was identified that maintenance technicians failed to properly restore and independently verify that a 22 AFW pump.

discharge pressure instrument isolation valve was in the open position following a July 7, 1998 calibration of the associated transmitter. This led to a failure to meet the Technical Specification 3.7.1.2 limiting condition for operation which requires that three AFW pumps be operable in operating modes 1, 2 and 3. Specifically, the steam generator injection line valves from the 22 AFW pump discharge would not have opened because the signal from the affected pressure transmitter was blocked.

This is a* Severity Level IV violation (Supplement I).

The NRC has concluded that information regarding the reason for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence and the date when full compliance was achieved is already adequately addressed on the docket in LER 50-311198-012-00and Supplement 01 dated August 24 and September 28, 1998, respectively. In that case, or if you choose to respond, clearly mark your response as a "Reply to a Notice of Violation," and send it to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

If you contest this enforcement action, you should also provide a copy of your response to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.

If you choose to respond, your response will be placed in the NRC Public Document Room (PDR). Therefore, to the extent possible, the response should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction.
