NRG INTEGRATED INSPECTION REPORT 50-272/98-09, 50-311 /98-09

NOTICE OF VIOLATION

Dear Mr. Keiser:

On October 24, 1998, the NRG completed an inspection at your Salem 1 & 2 reactor

facilities. The enclosed report presents the results of that inspection.

During the six-week period covered by this inspection report, your conduct of activities at

the Salem facilities was generally characterized by safety-conscious operations, sound

engineering and maintenance practices, and careful radiological work controls. The

inspectors performed a review of plant maintenance to assess the effectiveness of the

work management program and found that the corrective mainte.nance backlog remained

high, and that the program is still evolving, requiring additional management oversight to

ensure improvement. We also evaluated the effectiveness of system engineering to

support safe operation of the plant, and found the experience level of engineers to be

adequate, acceptable engineering training and procedures, and improved system

performance monitoring. ,

We are. concerned, however, about a violation of NRG requirements that was initially

documented in NRG Inspection Report 50-272,311 /98-06 dated August 21, 1998, where

your staff identified a failure to restore an auxiliary feed water pump runout pressure

transmitter to service following calibration. This resulted in the failure to detect an

inoperable pump for eighteen days. We have reviewed your licensee event report (LER) for

this event and concluded that corrective actions were reasonable and comprehensive.

However, because corrective actions for previous similar issues were not effective in

preventing recurrence, this violation was cited.

The NRG has concluded that information regarding the reason for the violation, the

corrective actions taken and planned to correct the violation and prevent recurrence is

already adequately addressed on the docket in the aforementioned LER 50-311/98-012-00

and Supplement 01 dated August 24 and September 28, 1998, respectively. Therefore,

you are not required to respond to this letter unless the description therein does not

accurately reflect your corrective actions or your position. In that case, or if you choose to

provide additional information, you should follow the instructions specified in the enclosed

Notice.
