NRC INTEGRATED INSPECTION REPORT 50-272/98-03, 50-311 /98-03

NOTICE OF VIOLATION

Dear Mr. Keiser:

On May 3, 1998, the NRC completed an inspection at your Salem 1 & 2 reactor facilities.

The enclosed report presents the results of that inspection.*

During the seven week period covered by this inspection, the of activities at the Salem

facilities generally involved safety-conscious operations, sound engineering practices, and

careful radiological work .controls. The prompt decision to reduce power and remove Unit

2 from service following an unplanned inoperability of a service water loop was viewed as

appropriate. Though maintenance was generally safe and controlled, we noted several

work coordination issues during emergency diesel generator on-line maintenance which led

to delays in restoring the equipment to operability.

On April 1, 1998, Confirmatory Action Letter 1-95-001 was modified and NRC approved

the restart of Salem Unit 1. Unit 1 achieved criticality on April 7, 1998. Overall, your

plant management and staff controlled the reactor startup, testing, and power ascension

activities well. The operating crews were attentive, used excellent communication skills,

and appropriately addressed all emergent issues and challenges.

In our letter dated March 17, 1998, we informed you that enforcement action from the

Readiness Assessment Team Inspection (RATI) would be addressed under separate

correspondence. The RATI identified issues involving the failure to follow procedures,

including the documentation of engineering evaluations and operability determinations, and

inadequate oversight of the Corrective Action Program. These violations are cited in the

enclosed Notice of Violation and the circumstances surrounding the issues were described

in detail in NRC Inspection Report 50-272, 311/98-81 sections M&03.2.b and E4.1.b

respectively. Please note that you are required to respond to this letter and should follow

the instructions specified in the enclosed Notice when preparing your response. The NRC

will use your response, in part, to determine whether further enforcement action is

necessary to ensure compliance with regulatory requirements .
