NRC INTEGRATED INSPECTION REPORT 50-272/96-06, 50-311/96-06
Dear Mr. Eliason:
On May 18, 1996, the NRC completed an inspection at your Salem I & 2 reactor
facilities. The enclos~d report presents the results of that inspection *. *
During the 6-week period covered by this inspection period, your conduct of
activities at the Salem facilities was generally characterized by safety-
conscious operations, sound engineering and maintenance practices, and careful
radiological work controls.
We are concerned, however, about a violation of
NRC requirements relating to poor procedure use and adherence by your
maintenance department.
The violation is cited in the enclosed Notice of Violation, and the
circumstances surrounding the violation are described in detail in the
enclosed report. Please note that you are required to respond to this letter
and should follow the instructions specified in the enclosed Notice when
preparing your response.
The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance
with regulatory requirements~
Acceptable actions were evident in the closure of several previously
identified electrical issues.
We continue, ~owever, to find instances where
the depth of evaluation was not sufficient to ensure the accuracy of your
conclusions. *
We found that PSE&G made progress in resolving the reliability and
configuration tontrol issues with the Hagan process instrumentation modules.
Not all issues pertaining to the Hagan modules, however, received the required
amount of attention. For instance, the modules operating temperature did not
receive the depth of review sufficient to ensure the accuracy of your
conclusions.
In addition, the design change packages for refurbished and
upgraded modules did not contain sufficient justification to demonstrate
acceptability of the safety evaluation and applicability of the design *
equivalency process. This restart issue remains open .
