SUBJECT:

SALEM AND HOPE CREEK COMBINED INSPECTION REPORT NOS. 50-272/95-03,

50-311/95-03, AND 50-354/95-02

Dear Mr. Eliason:

This letter refers to the announced safety inspection conducted by

Messrs. J. Calvert and J. Trapp of this office from February 4-17, 1995, at

Salem, Units 1 and 2 and Hope Creek Nuclear Generating Stations. The

inspectors discussed the findings of the inspection with members of your staff

at the conclusion of the inspection.

Areas examined during this inspection are described in the enclosed inspection

report. This inspection was focused on two separate activities. The first

week of the inspection was directed toward a review of Salem's actions taken

in response to the potential loss of an automatic engineered safety features

(ESF) equipment actuation signal, and the subsequent problems that occurred

when attempting to correct this deficiency. The second week of the inspection

focused primarily on the activities of the Hope Creek plant technical

department.

The inspection of engineering and technical support activities is

important to ensure that integrity and configuration of plant safety systems

are maintained in a condition ready to perform their intended safety function.

Within these areas, the inspection consisted of examinations of procedures and

representative records, interviews with personnel, and observations by the

inspectors.

The inspectors concluded that Salem's engineering response to the potential

for loss of an automatic ESF equipment actuation signal was timely and

technically sound.

Management acted promptly on the information presented to

them by the engineering staff. The engineering evaluation conducted following

the difficulties experienced with the installation of the design modification

to correct the problem was thorough and the corrective actions implemented

were appropriate.

In general, the Hope Creek technical engineering organization was determined

to provide strong technical support.

Newly developed initiatives, such as the

system readiness report and work-around list, were an indication of Technical

Engineering management's continued strong commitment to enhancing department

performance.

However, a general weakness was noted in the quality of root

cause evaluations. Several root cause analyses reviewed were poorly

documented, did not clearly identify a root cause for the failure, and did not

use root cause analysis techniques as required by your procedures.

inspectors noted that your staff was aware of this weakness and were making

significant changes to enhance the effectiveness of your corrective action

program.

A strong management commitment is necessary to assure the timely

implementation of any proposed changes.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of

this letter and its enclosure will be placed in the NRC Public Document Room.

You need not respond in writing to this letter. Your cooperation is

appreciated.
