NRC REGION I COMBINED INSPECTION REPORT NOS. 50-272/94-33 AND

50-311/94-33

Dear Mr. Eliason:

This letter transmits the report of the announced safety inspection conducted

by Ms. L. Harrison and Mr. G. Morris of this office for the periods of

November 30-December 2, 1994, December 7-9, 1994, and December 12-16, 1994, at

the Salem Nuclear Generating Station, located in Hancocks Bridge, New Jersey.

The inspectors discussed the findings of this inspection with members of your

staff on December 16, 1994.

This inspection reviewed actions taken to address four electrical-related

events occurring between November 11-28, 1994, and previously identified

unresolved items associated with the electrical distribution system functional

inspection (EDSFI) and power range neutron detection.

In addition, this

inspection evaluated the adequacy and installation of the reactor coolant pump

lube oil collection system for Unit 2.

The inspection was directed toward assessment of areas important to public

health and safety to determine whether activities authorized by the license

were conducted safely and in accordance with NRC requirements. Within these

areas, the inspection consisted of selected examinations of design

calculations, relevant procedures, representative records, installed

equipment, along with personnel interviews.

The inspectors concluded that equipment troubleshooting performed after the

four electrical-related events supported the root cause conclusions.

Corrective actions were found to be appropriate and should prevent similar

occurrences.

The expertise of plant personnel assigned to evaluate these

issues was appropriate.

The inspectors found that an oil collection system had been generally designed

and installed to collect oil from reactor coolant pumps on Unit 2 as described

in 10 CFR Part 50, Appendix R, Section 111.0.

However, a concern was

identified regarding the lack of a collection device for a certain plug and

flange as described in the attached report.

An evaluation prepared by your

staff does not consider these locations to be credible potential leakage

sites. Further NRC review is required to verify full compliance with the

requirements described in III.O. This concern will be tracked as an

unresolved item.

No significant safety concerns were identified. Minor examples of inattention

to detail were noted during review of previously identified EDSFI issues.

Specifically, calculation discrepancies existed regarding temperatures in

switchgear and penetration areas, and emergency diesel generator loading

recovery times.

Management was found to be actively involved in resolving open and emergent

technical issues.

The inspectors determined that management oversight was

evident in support of safe and reliable plant operation.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of

this~letter and its enclosure will be placed in the NRC Public Document Room.
