SALEM RESIDENT INSPECTION NOS. 50-272/94-31; 50-311/94-31

Dear Mr. Eliason:

The enclosed report documents an inspection for public health and safety,

conducted by Mr. C. Marschall, Senior Resident Inspector and other members of

the NRC resident and regional staff at the Salem Generating Station, for the

period between November 6, 1994 and December 17, 1994. The findings of this

inspection were previously discussed with Mr. J. Summers, General Manager-

Sal em Operations, and other members of your staff.

The report indicates performance improvement relative to the attention that

your staff is directing toward identifying and communicating degraded

equipment and system performance to appropriate management personnel. The

report also identifies improved interaction among your engineering,

operations, and maiDtenance organizations relative to the diagnoses and

resolution of equipment and system problems. -'

During this period, we also observed a "work-around" condition that your

organization willingly accommodated.

Specifically, in order to diminish the

number of fault alarms that could not be attributed to a specific system

fault, your operations organization deactivated the automatic test insertion

circuitry affecting the Safeguard Equipment Control (SEC) system.

While

active automatic test insertion circuitry is not required for operability of

the SEC, the action contributed to the need to initiate a controlled shutdown

of Unit 1 due to stuck test switches which affected SEC operability. The

condition was resolved before complete shutdown.

However, the situation did

require maneuvering of reactor power which may have been unnecessary if the

root cause of the frequent circuit fault alarms had been determined.

Within the scope of this inspection, the report also describes an violation

involving procedural adequacy which affected the testing of containment spray

system.

Since we consider that a basis for enforcement discretion exists in

accordance with NRC Enforcement Policy (10 CFR 2, Appendix C, VII.B), the

apparent violation is not cited.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of

this report and its enclosures will be placed in the NRC Public Document Room.
