SALEM INSERVICE TEST PROGRAM INSPECTION 94-21

Dear Mr. Eliason:

This letter refers to the safety inspection conducted by Mr. D. Dempsey of

this office, and Mr. J. Colaccino of the Office of Nuclear Reactor Regulation,

from September 12-16 and October 19-21, 1994, at the Salem Nuclear Generating

Station, Hancocks Bridge, New Jersey.

The inspection focused on issues

important to public health and safety, specifically your implementation of an

inservice test (IST) program for pumps and valves pursuant to 10 CFR 50.55a(f)

and NRC Generic Letter 89-04,

"Guid~nce On Developing Acceptable Inservice

Testing Programs."

At the conclusion of the inspection, the preliminary

findings were discussed with Mr. S. LaBruna and other members of your staff.

In general, our inspection found that the Salem IST program met regulatory

requirements.

We observed good practices related to the use of spectrum

analysis for vibration and high quality procedures.

However, we identified

several shortcomings which, in their totality, indicate the need for more

effective oversight of the program by station management.

We are concerned

particularly that many issues similar to those discussed in this report have

been identified previously in comprehensive and self-critical audits performed

by your quality assurance organization, but have not been acted upon.

Furthermore, NRC program reviews have also identified similar uncorrected

deficiencies.

Certain of your activities appear to be in violation of NRC requirements, as

specified in the enclosed Notice.

We found one instance in which two safety-

rel ated check valves in the suction line to the charging pumps were not tested

in accordance with the requirements of Section XI of the ASME Boiler and

Pressure Vessel Code.

As a result of this finding, you subsequently

identified other check valves which similarly were not tested. Also we

identified two instances, involving post-maintenance pump baseline data

verification and premature lifting of relief valves during surveillance

testing, where your corrective action program was not effective in precluding

recurrence of conditions adverse to quality.

You are required to respond to

this letter and should follow the instructions specified in the enclosed

Notice when preparing your response, including actions you plan to take to

prevent recurrence. The responses directed by this letter and the enclosed

Notice are not subject to the clearance procedure of the Office of Management

and Budget, as required by the Paperwork Reduction Act of 1980, Pub. L. No .

96.511.
