SALEM RESIDENT INSPECTION NOS. 50-272/94-24; 50-311/94-24
Dear Mr. Eliason:
The enclosed report documents an inspection for public health and safety,
conducted by Mr. C. Marschall, Senior Resident Inspector, Salem, and other
members of the NRC staff at the Salem for the period between September 18,
1994 and November 5, 1994.
The inspectors discussed the findings of this
inspection with Messrs. J. Hagan of your staff.
Based on the results of this inspection, it appears that certain of your
activities were not conducted in full compliance with NRC requirements, as set
forth in the Notice of Violation. These violations indicate continuing
weaknesses relative to prompt and critical assessment of plant conditions,
procedure adherence, and corrective action effectiveness.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence.
Your response may reference or
include previous docketed correspondence, if the correspondence adequately
addresses the required response. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
We are also concerned with the apparent recurrence of several work practice or
control problems that had the potential to cause severe personnel injury or
.compromise personnel safety.
We were particularly concerned about the
circumstances involving a contractor electrician who inadvertently cut into
the wrong 4160 VAC cable; and note that it was fortuitous, and not by design,
that the cable was de-energized at the time.
While none of the events
compromised nuclear safety, the recurrence of items, similar in nature to
events which formed the basis of previous NRC enforcement action in March
1994, appears to indicate continued ineffective corrective action
implementation.

assessment of this particular concern and identify any actions, taken or
planned, relative to improvement .
