APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Units 1 and 2 Hope Creek Generating Station Docket Nos:

License Nos:

50-272 50-311 50-354 DPR-70 DPR-75 NPF-57 During an NRC inspection conducted on June 26, 1994 - August 6, 1994, two violations of NRC requirements were identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1992), the violations are listed below:

A.

Hope Creek Technical Specification 3.3.7.4 requires, in part, that remote shutdown system controls shall be operable in Operating Conditions 1 and 2.

Operability of the Remote Shutdown Panel requires the ability to remotely operate valve BC-HV-F004A (the "A" residual heat removal suppression pool suction valve).

Contrary to the above from July 25, 1986 (issuance of the license for Hope Creek operation) until July 7, 1994, the remote shutdown system control for valve BC-HV-F004A, which effects the remote operation of the valve from its remote shutdown control station, was not operable in that the valve could not be properly controlled from the remote operating switch in accordance with plant design.

This is a Severity Level IV violation (Supplement I).

10 CFR 50, Appendix B, Criterion 11 requires, in part, that licensees shall provided training of personnel performing.activities affecting quality as necessary to assure suitable proficiency is achieved and maintained.

Contrary to the above, on July 13, 1994, the licensee did not provide the training necessary to assure suitable proficiency for personnel assigned to change the oil in the governor gearbox for the Salem Unit 2 no. 23 auxiliary feedwater pump.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit a written statement or explanation to the U.S. Nu.clear Regulatory Commission, ATTN:

Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a "Reply to a Notice of*

Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the 9409260017 940919 PDR ADOCK 05000272 G

2 corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued to show cause why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
