SUBJECT:

SALEM AND HOPE CREEK RESIDENT INSPECTION NOS. 50-272/94-14;

50-311/94-14; 50-354/94-13

The enclosed report documents an inspection for public health and safety,

conducted by Mr. C. Marschall, Senior Resident Inspector and other members of

the NRC resident and regional staff at the Salem and Hope Creek Nuclear

Generating Stations for the period between June 26, 1994 and August 6, 1994.

The inspectors discussed the findings of this inspection with Messrs. J. Hagan

and R. Hovey of your staff.

Based on the results of this inspection, it appears that two of your

activities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Vi o 1 at ion. * Re 1 at i ve to Hope Creek, our inspectors

determined that the "A" RHR suppression pool suction valve did not function,

as designed, relative to the ability to control the valve from the remote

shutdown panel; and that though previous surveillance activities detected the

deficiency, effective corrective action was not established until July 1994.

Relative to Salem Unit 2, maintenance activities associated with an oil change

involving the No. 23 Auxiliary Feedwater pump, a safety related component,

were erroneously performed due to insufficient training of the maintenance

technicians and inadequate management oversight and control. These examples

may be indic~tors of weaknesses in your programs affecting maintenance and

surveillance activities. A contributing factor, in both these cases, appears

to involve the management, oversight, and control of the specific activities.

Accordingly, you are required to respond to these apparent violations in

accordance with the instructions provided in the enclosed Notice of Violation.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice, a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Public Law No. 96.511. Within the

scope of this inspection, the inspectors also noted one non-cited violation

relative to the improper control of an effluent release of processed liquid

radioactive water.

No reply is required for this issue.

The letter also acknowledges receipt of your May 18, 1994 response to a Salem

violation cited in the NRC Inspection Report 50-311/94-06.

We will evaluate

your corrective actions in a future inspection of your licensed program.
