SALEM AND HOPE CREEK RESIDENT INSPECTION NOS. 50-272/93-19;

50-311/93-19; 50-354/93-18

The enclosed report documents an inspection to assure public health and safety, conducted by

Mr. T. Johnson-Senior Resident Inspector and other members of the NRC resident and

regional staff at the Salem and Hope Creek Nuclear Generating Stations for the period

between June 6, 1993 and July 24, 1993. The inspectors discussed the findings of this

inspection with Messrs. C. Vondra and R. Hovey of your staff.

During the course of the inspection period, the inspectors concluded that both the Salem and

Hope Creek facilities were operated in a safe manner. However, several events occurred at

Salem During the period which indicated lapses in your staff's performance at those two

units. These events included two reactor trips, an ammonia gas release, a trip of a vital

electrical bus, a service water pump tagging error, and the failure of a main condenser

-radiation monitor. The root causes of the events generally involved inappropriate

management or supervisory oversight, inadequate corrective actions, and lack of attention to

detail or procedural adherence. These types of causes are similar in nature to deficiencies

noted in previous NRC inspections. While none of these matters was identified as a

violation, we*continue to be concerned with your ability to improve performance. While we

are aware of your efforts to assess previous performance deficiencies, as discussed in our

meeting on July 16, 1993, please respond within 30 days of the date of this letter describing

actions (taken or planned) that are designed to reverse this apparent trend in performance.

In accordance with 10 CFR 2. 790 of the NRC's "Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room. The response directed by

this letter is not subject to the clearance procedures of the Office of Management and Budget

as required by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
