Combined Inspection Nos. 50-272/93-11, 50-311/93-11 and 50-354/93-07
This letter refers to the Fitness-for-Duty (FFD) Program inspection conducted by
Mr. R. J. Albert of this office on April 5-7, 1993, of activities authorized by NRC License Nos.
DPR-70, DPR-75 and NPF-57 and to the discussions of our findings held with Ms. M. Butz and
other members of your staff by Mr. Albert at the conclusion of the inspection.
Areas examined during this inspection are important to public health and safety and are
described in the NRC Region I inspection report which is enclosed with this letter. Within these
areas, the inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, and observations by the inspector.
Overall, we found that your FFD program was directed toward public health and safety.
Continued good management support of the program was evident. However, we are concerned
that a commitment made in your January 29, 1991, letter, in response to our concerns about
personnel with infrequent unescorted station access, could not be met for various reasons, but
you did not notify us or request relief from the commitment.
Your failure to meet that
obligation is a deviation from a written commitment. In addition, your failure to take effective
corrective measures for a recurring programmatic problem in FFD training for supervisors with
responsibilities for implementing certain aspects of the FFD program is a violation of regulatory
requirements.
As set forth in the Notice of Deviation, enclosed herewith as Appendix A, and the Notice of
Violation, enclosed herewith as Appendix B, you are requested to respond to Appendix A and
required to respond to Appendix B.
In preparing your responses, you should follow the
instructions in the Appendices.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures will be placed in
the NRC Public Document Room.
