r ENCLOSURE 1 NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Nuclear Generating Station Docket Nos: 50-272 50-311.

Units 1 and 2 Hope Creek Generating Station 50-354 License Nos: DPR-70 DPR-75 NPF-57 As a result of the inspection conducted from February 23-26 and March 30-31; 1993, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Action," 10.CFR Part 2, Appendix C, 1992, the following violation was identified:

10 CPR 50.59(b)(l) states, in part, that records of changes to the facility as described in the updated final safety analysis report (UFSAR)"... must include a written safety evaluation which provides the basis for the determination that the change, test, or.

experiment _does not involve an unreviewed safety question."

Contrary to the above, the licensee made the following changes to the facility as described in the UFSAR and did not provide written safety evaluations providing the basis for a determination that an unreviewed *safety question was not involved.

Hope Creek.Deficiency Report*HTE 92-230 documented a use-as-is disposition

  • for unqualified gauges in the gland Seal portion of the high pressure coolant
  • injection (HPCI) system, including changing the normal _position of the isolation valves for these gauges from open to closed, which changed the facility as described in UFSAR Figure 6.3,.2 because* it shows the gauges within the safety-related boundary and the isolation valves as being normally.

Salem Unit 1 Temporary ModificationTMR 92-031 provided temporary.power

  • from the lC vital bus (normal power if from the lB vital bus) to the Salem 1 No. 12 Auxiliary Building fan, which changed the facility as described in.

Tables 8.3-2, 8.3-3, and Figure 8.3-4A in the UFSAR.

Salem Umt 2 _Temporary Modification TMR 92-043 installed a temporary blankJlange in the service water system upstream of manual isolation valve 22Sw'414, which changed the facility as shown on Figure 9.2-lB in the UFSAR.

These three examples are collectively a Severity Level IV Violation (Supplement I).

2 Pursuant to the provisions of 10 CPR 2.201, Public Service Electric and Gas Company is hereby required to submit a written statement of explanation to the U.S. Nuclear Regulatory Commission, ATT: Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region I, and, a copy to the NRC Resident Inspector, within 30 days of the receipt date of the letter whiCh transmitted this Notice of Violation. This reply should be clearly marked as "Reply to a Notice of Violation" and should include: (1) tlie reason for the violation, or, if contested, the basis for disputing the violation; (2) the corrective steps that have been taken and the results achieved;. (3) the corrective steps which will be taken to avoid further violations;. arid ( 4) the date when full compliance will be achieved. Where good cause is shown, consideration will. be given to extending. this response time.
