This letter forwards the final SALP reports for the Salem and Hope Creek Gene~ating _

  • stations for the period between August 1, 1990 and December 28, 1991 (Enclosures l(a) and

l(b)) .. The.initial SALP reports were enclosed in our correspondence to you dated March 25,

1992 (Endosure 2). We discussed the SALP reports in a open meeting with you and your

staff at the Artificial Island Processing Center, Bancocks Bridge, New Jersey on April 15,

1992.

Based on the discussions at that meeting and your written response dated April 29, 1992

(Enclosure 3), we have revised the Salem final SALP report to reflect the minor correction of

certain details, as you indicated, relative to the weakness in. the description of the emergency

action levels on Page 18, and clarification on Page 28, thatPSE&G, instead of a vendor, did

. the chloride hide-out calculation (Enclosure 4 pertains). No substantive changes were made .

from the initial version of either SALP report.

Our overall assessment indicated that your organization maintained an acceptable level of

performance that was generally consistent with the previous performance noted for Salem ~d

Hope Creek. We .noted that your organization was particularly challenged by the Salem Unit

2 Turbine/Generator failure. While the event revealed weaknesses in certain personnel *

performance, preventive maintenance progrctms, and commitment tracking processes, your

staff displayed a high degree of competence and ability relative to root cause investigation

and analyses, and recovery from the event.

. Relative to Salem, your organization has significantly improved the material condition of the

plant. Though declining in frequency, instances of personnel error, insufficient adherence to

procedures, and*inattention to detail, continue to require aggressive management attention and

. oversight to assure effective resolution.

Relative to Hope* Creek, strong performance and aggressive management were noted relative

to self-assessment efforts, corrective action implementation, and root cause analyses, As a

result, your organization's performance in operations, radiological controls, emergency

preparedness, security, and safety assessment and qu3.lity .Verification were regar:ded as*

superior. However, significant weakness was noted relative to management attention and

  • communication on certain generic matters such as analysis of motor-operated valve .

performance, and requirements relative to station black-out. We believe that your

assessments and communications relative to generic concerns require continued management

. attention and oversight to assure improved quality.

,

As discussed in the management meeting* and in your subsequent response to our initial SALP

reports, we understand that you have ongoing activities and corrective actions in place to

address the identified weaknesses and effect improvement in performance. your actions will

be reviewed during future inspections_ of your facility and programs.
