Docket Nos. 50-272

50-311

JUN 1 3 1991

  • Public Service Electric and Gas Company

ATTN:

Mr. Steven E. Miltenberger

Vice President and

Chief Nuclear Officer

P. 0. Box 236

Hancocks Bridge, New Jersey 08038

Gentlemen:

Subject: NRC Safety System Functional Inspection Team Report Nos. 50-272/91-80 and

50-311/91-80

This letter forwards the report of the safety system functional inspection (SSFI) performed by

an NRC team from April 15 to 26, 1991, at the Salem Nuclear Generating Stations. The

team's findings and preliminary conclusions were summarized at the exit meeting held at the

Salem site on April 26, 1991, and were further discussed with your staff in a telephone

conversation on May 10, 1991.

The objective of the SSFI was to assess the operational capability of the residual heat removal

(RHR) system to perform its design basis safety function. * The team evaluated the adequacy

of operational procedures, test practices, and maintenance policies as they contribute to the

system's reliability. The team also addressed the quality of design control and other

management programs applied to the systems.

The team concluded that, based on the areas of observation, the systems were capable of

performing their design basis safety functions.

It appears, however, that two activities were not conducted in full compliance with NRC

requirements, as set forth in the Notice of Violation enclosed herewith as Appendix. A. The

violations have been categorized by severity level in accordance with the "General Statement

of Policy and Procedure for NRC Enforcement Actions," lOCFR Part 2, Appendix C

(Enforcement Policy). It is further noted that the above two violations had similar

deficiencies in that they both failed to consider the worst case conditions* which led to

nonconservative analyses. You are required to respond to this letter and in preparing your

response, you should follow the instructions in Appendix A .

The violations and weaknesses identified by the inspection team are summarized in the

executive summary. Weaknesses are conditions presented for your review to establish what,
