Box 236 Hancocks Bridge, New Jersey 08038 Gentlemen: Subject: Inspection No. 50-272/90-20; 311/90-20 This letter transmits the findings of the inspection conducted by Messrs. T. P. Johnson, S. M. Pindale, S. T. Barr, and H. I. Gregg from June 28 - August 15, 1990 at the Salem Nuclear Generating Station. The findings of the inspection were discussed with Mr. V. J. Polizzi of your staff. The inspection was conducted to review the circumstances surrounding the mis- operation of two out of four main steam isolation valves .(MSIVs) following a Unit 2 automatic reactor trip on June 28, 1990. Subsequent review identified an apparent main steam line isolation circuit design deficiency which caused the failure of the two MSIVs to fully close when manually actuated. Additional MSIV problems related to the valves* ability to fully close under certain postulated conditions were identified and resolved. No reply to this letter is required.
