LaSalle U2 - LER 2017-004 NRC Information Request- Mel Holmberg (03-29-18)

For the affected Safety Relief Valves (SRVs):
- 1. Corrective action (CA) records and cause investigations (including vendor cause investigations) that relate to the SRV lift setpoint failures for LER 2017-004-001 and for LER 2015-002-01.
- 2. Date that each of the affected SRVs was placed in service (since prior successful lift test) and dates of the operating cycles that each valve was in-service prior to failed lift test.
- 3. Lift test dates for each affected valve for the current and previous two surveillance tests and identify the test vendors.
- 4. Identify scope expansion (if any) for SRVs that occurred as a result of these failed lift tests and/or any evaluations of the need for scope expansion.
- 5. Completed as-found and as-left lift test records for current failed tests and prior test (e.g.
completed Surveillance Tests (last two).
- 7. P&ID Drawing identifying location of affected SRVs on the Main Steam Piping.
- 8. Design Specification for affected relief valves.
- 9. Applicable Code or Standard for Valve Design and Construction.
- 10. Vendor Technical Manual.
- 11. NSSS and valve vendor Safety Information Letters (SILs) or similar vendor documents applicable to these valves.
- 12. List with description of Modifications made since original installation.
- 13. List with description of completed Maintenance Work orders for affected valves for past two inservice operating cycles.
- 14. History of SRV lift setpoint changes and how these changes have been controlled including any calculations that determined lift setpoint, reseat and blowdown tolerances.
- 15. Safety Relief Valve Simmer Margin Analysis - The difference between normal plant operating pressure and the SRV opening setpoint.
- 16. Inservice Testing Program and Basis Document (Applicable Portions for SRVs).
- 18. Maintenance Procedures including vendor procedures last used to refurbish the affected valves prior to lift test failure.
- 19. ASME Code Section XI repair/replacement Plan, and ASME Section XI - NIS-2 Form for last time valve was replaced.
- 20. Provide a copy of the site program and procedures related to surveillance testing of these SRVs.
- 21. Provide a copy of the applicable edition of the basis document governing testing of the affected valve (e.g. ASME/ANSI PTC-25.3 or the ASME OM Code - Code for Operation and Maintenance of Nuclear Power Plants).
- 22. Docketed correspondence related to SRV testing (e.g. relief requests and NRC evaluation of these relief requests and associated code cases - Code Case OMN-17).
