APPENDIX A NOTICE OF DEVIATION Public Service Electric & Gas Company Salem Generating Station, Units 1 and 2 Docket Nos.
50-272/50-311 License Nos. DPR-70/DPR-75 As a result of the inspection conducted during the period of May 22 through May 26, 1989, and in accordance with the 11General Statement of Policy and Procedure for NRC Enforcement Actions, 11 10 CFR Part 2, Appendix C (Enforcement Policy) (1988), the following deviations were identified:
Table 7.5-3 of the Salem Generating Station FSAR classifies the variables listed below as type A with Category 1 requirements.
In addition, Tables 7.5-4 and 7.5-5 indicate that the same variables either comply or would be upgraded to comply with the criteria of Regulatory Guide 1.97.
Variables' Description Reactor Coolant System Hot Leg Water Temperature Effluent Radioactivity Monitor for Noble Gas from Condenser Air Removal System Exhaust Auxiliary Feedwater Fl ow Steam Generator Radiation For Category 1 inst,rumentation, Section-1.3.1.b of Regulatory Guide 1.97, Revision 2, states:
11 No single failure... should prevent the operators from being presented the information necessary for them to determine the safety status of the plant... Where failure of one accident monitoring channel results in information ambiguity... additional information should be provided... This may be accomplished by providing additional independent channels of information of the same variable... or by providing an independent channel to monitor a different variable that bears a known relationship to the multiple channels.
Redundant or diverse channels should be electrically independent and physically separated from each other... 11 Contrary to the above, at the time of the inspection, the variables identified above did not meet the redundancy criteria in that no redundant or diverse channels were provided to ensure that adequate post-accident monitoring instrumentation is availabl~ to the operators.
This is a deviation.
Please, provide to the U.S. Nuclear Regulatory Commission, Attn:
Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator and a copy to the NRC Resident Inspector, in writing within 30 days of the date of this Notice, the reason for the Deviation, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further Deviations, and the date when your corrective actions will be completed.
