Special Inspection No. 50-272/89-16; 50-311/89-15

This letter transmits the findings of a ~pecial inspection conducted by Mrs.

Kathy Halvey Gibson and Mr. Stephen M. Pindale from May 27, 1989 to July 10,

1989, at the Salem Nuclear Generating Station.

The findings were discussed

with Mr. L. K. M1ller of your staff at the conclusion of the inspection.

This -inspection reviewed the circumstances surrounding inadequate response time

testing of the isolation function of the main and bypass feedwater control

valves, and that when the feedwater control valves were properly tested,

response times did not meet Technical Specification requirements.

As such, if

the valves had been called upon to close, feedwater isolation may not have

occurred within the time assumed in the accident analysis.

Consequently, we have scheduled an Enforcement Conference at the NRC Region I

office on July 26, 1989, to discuss this matter.

At the conference you should

be prepared to discuss the causes of the event, your response including

corrective actions, and the safety significance, including any potential

broader implications.

Specifically, you should be prepared to discuss your

philosophy and the potential safety impact regarding the failure to establish

a~ as-found condition for the feedwater control valves.

In addition, you

should identify any similar situations or safety equipment for which you do not

establish an as-found condition, and be prepared to discuss the safety

implications, if any, of those situations identified.

Following the

conference, we will notify you of our decision regarding any additional

enforcement action.
