This letter transmits the findings of the routine,

conducted by Mrs. K. Halvey Gibson and others from

Based on the results of this inspection, it appears that certain of your

activities were not conducted in full compliance with NRC requirements, as set

forth in the Notice of Violation, enclosed herewith as Appendix A.

You are

required to respond to this letter and in preparin~ your response, you should

follow the instructions in Appendix A and as delineated in this letter.

The first violation deals with three instances of procedural noncompliances

which resulted in safety system actuations during the report period.

In your

response, please describe any pertinent conditions such as plant status,

control room activities, or number of hours worked which may have contributed

to the errors.

Where such conditions are identified, please indicate the

actions you plan to take to eliminate those conditions.

The second violation involves your failure to correct or prevent recurrence of

self-identified material control nonconformances for over a three year period.

We are concerned that such nonconformances continued during this period

despite recurring quality assurance findings and implementation of escalation

procedures required by the Quality Assurance Program.

Consequently, in your

response to the violation please provide your evaluation of whether this

problem represents a breakdown of your Quality Assurance Program.

We are also

concerned that due to your failure to correct the identified problems, the

number of nonconformances identified, and the apparent lack of pre-issue

controls to verify the appropriateness of materials issued during this three

year period, nonconforming material could potentially have been installed in

safety-related systems.

Please address this concern in your response indicating

what steps you plan to take to determine whether nonconforming material has

been used in plant systems, and what you will do to prevent recurrence.
