Combined Inspection Numbers 50-272/88-80 and 50-311/88-80
This refers to the special safety inspection conducted by D. L. Caphton of
this office on October 17-28, 1988, of activities authorized by NRC License
Nos. DPR-70 and DPR-75 at the Salem Nuclear Generating Station, Units 1 & 2
and at the Salem training simulator in Salem, N.J., and to the discussions of
our findings held by Mr. Caphton with Mr. L. Miller and other members of your
staff at the cqnclusion of the inspection.

Areas examined durin*g this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter. Within these areas, the
inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, and observations by the inspector.
Within the scope of this inspection, no violations were observed.
However, the
inspection identified a number of concerns that, when viewed collectively,
indicate that an increased level of management involvement, control and
attention to detail is needed relative to the design change and modification
process. These concerns are listed in the Attachment C to the report along
with the reference to the pertinent report paragraph.
You are requested to
address the overall concern regarding management involvement, control and
attention to detail and provide a response within thirty days of the date of
this letter delineating corrective actions planned or taken.
Our inspector also verified the steps you have taken to correct the violations
brought to your attention *in our letters dated February 25, 1987 and July 17,
1987.
We have no further questions regarding the steps you took to correct
Items 1 and 2 in Appendix A of the July 17, 1987 letter. With regard to Items
B and C in Appendix A of the February 25, 1987 letter, these items were not
fully corrected.
We noted that you are taking steps to correct these items; in
your response to this letter, please notify this office of the expected
completion dates.

have been falsified.
Pending the completion of your investigation and review
by the NRC, this matter is designated an unresolved item (reference paragraph
3.5 in the enclosed report).
