Subject: NRC Combined Inspection Report No. 50-272/88-18; 50-311/88-18
A routine radiological controls inspection was conducted by Mr. R. L. Nimitz at
the Salem Nuclear Generating Station on* September 26-30 and October 5-6, 1988.
Areas reviewed during the inspection included action on previous NRC findings,
organization and staffing, training and qualifications, corrective action
systems, external and internal exposure controls, ALARA and worker concerns.
The findings of the inspection were discussed with Mr. S. LaBruna on September
30, 1988 and Mr. L. Miller on October 6, 1988.
Within the s.cope of this inspection, one apparent violation of NRC requirements
was identified and is discussed in the enclosed Appendix A, Notice of Violation.
The violation involves a number of examples of failure to implement radiation
protection procedures.* The insp~ction also identified a number of weaknesses in
the supervisory and management oversight of ongoing radiological work
activities, weaknesses in your industrial safety program and weaknesses in the
corrective action system for radiological controls deficiencies.
You are required to respond to fhe enclosed Notice of Violation and sho~ld
follow the instructions contained therein.
In your response, please describe
your actions taken or planned to improve the supervisory and management
oversight of radiological work activities and improve the corrective action*
process for identified deficiencies.
