DESCRIPTION This design change added weep holes.in the valve disk on the upstream side of Valves SJ12 and SJ13.
This provides a thermal expansion path for fluid trapped between the disks and prevents hydraulic locking of the valve.

SAFETY EVALUATION 10 CFR 50.59 As a result of the weep hole addition and relief line removal, the valve configuration for the safety injection system will be altered to allow the SJ4 and SJ5 Valves to remain open.
This line up will ensure the BIT is always full and will reduce the number of valves required to open on a safety injection signal.
This modification did not alter any plant process or discharge or affect the environmental impact of the plant.
No unreviewed safety or environmental questions are involved.

SALEM UNIT NO. 1 SALEM GENERATING STATION MONTHLY OPERATING
SUMMARY
UNIT NO. 1 AUGUST 1988 The Unit began the period operating at 100% power and continued to operate at that level until August 20, 1988, when a Unit shutdown was commenced.
The shutdown was initiated due to repairs to No. 12 Charging Pump not being completed within the Technical Specification time limit.
Later the same day, the charging pump was restored to operable status and the shutdown terminated at 38% power.
The Unit was restored to full power on August 21, 1988, and continued to operate at that level until August 31, 1988, when a turbine/reactor trip occurred due to a malfun.ction of the turbine hydraulic system during a surveillance test of the vacuum trip function.
The Unit remained shutdown for the rest of the period.
