APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Unit 2 Docket No.
50-311 License No. DPR-75 As a result of the inspection conducted on May 3, 1988 - June 6, 1988, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions, 11 10 CFR 2, Appendix C (Enforcement Policy 1986), the following violation was identified:
A.
Technical Specification (T.S.) Surveillance 4.0.5 states that surveillance requirements for inservice inspection and testing of ASME Code Class 1, 2 and 3 components shall be applicable as follows:
Inservice inspection of ASME Code Class 1, 2 and 3 components and inservice testing of ASME Code Class 1, 2 and 3 pumps and valves shall be performed in accordance with Section XI of the ASME Boiler and Pressure Vessel Code and applicable Addenda as required by 10 CFR 50, Section 50.55a(g), except where specific written relief has been granted by the Cammi ssi on pursuant to 10 CFR 5.0, Section 50.55a(g)(6)(il.
T.S. 6.8.lc requires that written procedures shall be ~stablished, implemented and maintained covering surveillance and test activities of safety related equipment.
Surveillance Procedure SP(0)4.0.5 V-MISC-1, Inservice Testing - Valves Miscellaneous," provides instructions and checkoff sheets for the performance of T.S. *surveillance 4.0.5 for Fire Protection Containment Isolation Valve 2FP147 and others. This surveillance is required to be performed every 92 days.
Contrary to the above, on April 26, 1988, it was determined that on February 10, 1988 Surveillance Procedure SP(0)4.0.5 V-MISC-1 was not properly implemented in that T.S. Surveillance 4.0.5 for Fire Protection Contafnment Isolation Valve 2FP147 was not completed within the previous 92 days.
This is a Severity Level IV violation (Supplement 1) applicable to DPR-:-75.
-Public Service Electric and Gas Company is not required to respond in writing to this Notice of Violation since corrective actions for this violation have been reviewed and found acceptable and full compliance had been achieved prior to issuance of.this Notice.
